ELIGIBILITY
The onsite clinic is available for use by active Full-time and Part-time Associates, spouses, domestic partners and dependent children between the ages of 8-26.
Associates, spouses, domestic partners and dependent children must be able to show proof of medical insurance to use the clinic; any full medical insurance plan is acceptable. Grande reserves the right to discontinue an individual’s privilege to utilize the Health & Wellness Center at any time.
UTILIZATION
- Associates should make every effort to schedule in advance their preventative or routine appointments on their days off or during the least disruptive time of their work schedule
- Associates will need to change out of their uniforms and boots before going into the clinic
- If an Associate has an illness while at work and needs to see the Nurse Practitioner, there will be same day appointments available; they would be paid during their appointment; however, manager approval is needed
- In the event that you need to travel to another Grande location for services, you must first clock out, change out of your facility uniform, boots and any other PPE before leaving. Associates will not be paid for mileage or time spent traveling to the other location. Once you return to your facility after the appointment, clock into the Kronos timekeeping system, change into your facility uniform, boots, PPE and return to work
HEALTH PROMOTION AND PATIENT EDUCATION
Management of Chronic Diseases:
Includes acne, anxiety, asthma, depression, diabetes, high blood pressure, high cholesterol, migraines, thyroid disorders and allergies.
Diagnosis and Management of Acute Non-Emergent Concerns:
Includes bronchitis, ear aches, fever, pneumonia, rashes, STDs, sinus infections, sore throat, sprains, strains, stomach concerns and urinary tract infections.
Lab Services:
Includes basic metabolic panel, complete blood count, comprehensive metabolic panel, hemoglobin A1C, lipid panel, mono test, pregnancy test, rapid strep A, pap with human papilloma virus, prostate specific antigen, sexually transmitted disease, thyroid stimulating hormone, urine dipstick and vaginal infection testing.
