Udlis graduated from the University of Western Ontario, Canada. He practices family medicine at the Fond du Lac Regional West Clinic and also oversees the Grande Health and Wellness Center as the Medical Director. Udlis has been with SSM Healthcare for 15 years.
SARAH DAVIS, NURSE PRACTITIONER

Sarah earned a Bachelor of Science degree in Nursing in 2002 and a Master of Science degree in Nursing Education in 2009 from the University of Wisconsin-Oshkosh. She recently completed her Family Nurse Practitioner from Alverno University-Milwaukee. Sarah is in the process of completing her Doctorate of Nursing Practice from Marquette University-Milwaukee. Sarah has had over 16 years of hospital RN experience and over 12 years of teaching experience. She currently teaches at Marquette University in both the undergraduate and graduate nursing programs. Sarah is excited to provide compassionate care to Grande associates and their families. Her goal is to educate patients on how to achieve a healthy lifestyle and reduce the risk of developing chronic illnesses.
MEGAN DIENER, NURSE PRACTITIONER

Megan earned a Bachelor of Science degree in Nursing from the University of Wisconsin-Oshkosh. She recently earned a Master of Science degree as an adult nurse practitioner from Marian University. Diener hopes to maintain the team approach in guiding and supporting the patients she cares for with a strong belief in teamwork between the patient and provider.
HILLARY BOEKE, MEDICAL ASSISTANT

Hillary graduated from Globe University-Middleton with a Medical Assistant degree in 2010. She is excited to be a part of the SSM Work & Wellness team to promote health and wellness to Grande Associates. She lives on a working dairy farm with her family. In her free time, she loves to help out on the farm and spend time with her family.
Natalia Chapman, PA-C

Natalia earned her bachelor’s degree in biology from the University of Wisconsin-Platteville. She received her Master of Physician Assistant Studies degrees from the University of Wisconsin-Madison School of Medicine and Public Health. She is NCCPA certified and a member of the Wisconsin Academy of Physician Assistants and the American Academy of Physician Assistants.
Natalia has four years of experience at SSM Monroe Clinic providing medical care in both the primary care and urgent care settings. Natalia also has a background in Education where she spent three years long-term and short-term substitute teaching at the elementary and high school levels.
Through this background, she prides herself in effectively educating her patients about their health, their treatment options, and accomplishing their healthcare goals and needs. She encourages her patients to have an active role in their health care.
She is looking forward to working with the Grande Associates and their families, with a focus on establishing a trusting rapport, in order to provide more comprehensive and inclusive healthcare.

Stefanie graduated from the University of Wisconsin-Madison with a Bachelor of Science degree in Nursing and earned her Master of Science Nursing degree as a Family Nurse Practitioner from University of Wisconsin-Oshkosh. She has over 5 years of experience in women’s health, occupational health, and acute and chronic disease management. She looks forward to working with Grande Associates to achieve their wellness goals and making healthier lifestyles for themselves.
JENNIFER KONRAD, NURSE PRACTITIONER

Jen earned her Bachelor of Science in Nursing in 1996 from Marian University in Fond du Lac Wisconsin and a Master of Science in Nursing degree in 2022 from Concordia University, Wisconsin. She is a board-certified Family Practice Nurse Practitioner with 26 years of healthcare experience. Jen is excited to join the Grande Health and Wellness team and passionate about helping patients meet their health and wellness goals.
Tina Smedema, Medical Assistant

Tina came to SSM after a COVID hospitalization at SSM St. Agnes Hospital. She received superb care and was treated with such kindness during her hospital stay. She left her job of 9 years to come to SSM. Tina looks forward to meeting everyone and helping everyone meet their health and wellness goals. Tina lives in Waupun with her 2 daughters. She enjoys hunting, traveling, listening to bands/music, and chasing her girls around with their travelling softball and hockey teams.
ROBERTA “BOBBI” LAST, SUPERVISOR/ CASE NAVIGATOR

Bobbi Last is the Manager for the Grande Health & Wellness Clinics. She started with the clinics as the RN case manager. Her entire career has been in the healthcare field, beginning at the age of 15 as a nursing assistant and continuing as a graduate of Marian College, now Marian University of Fond du Lac. Last has 24 years RN experience and 11 years of case management experience. Bobbi’s passion is assisting people to function at peak capacity and enabling clinic staff to serve our client and patients with excellence.
DIANE HALLMAN, REGISTERED MEDICAL ASSISTANT

Diane has been a Medical Assistant for over 14 years after graduating from Fox Valley Technical College. The majority of her experience has been in family practice. As part of the SSM Work and Wellness team at Grande, she is excited to assist in empowering patients to make positive lifestyle changes to better their overall quality of life.
Darcey Amundson, Physician Assistant

Darcey graduated from Illinois State University with a Bachelor of Science degree in Medical Technology. She received her Physician Assistant degree from Western Michigan University. She is nationally certified by the NCCPA. She has 20 years of experience as a Physician Assistant in several areas of primary care including urgent care, family medicine, occupational health, and most recently employee-based wellness. She looks forward to working with Grande Associates and their families to make positive healthy lifestyle choices, providing patient education, and empowering patients to be in charge of their health.
Brianna Agee, Medical Support Specialist

Brianna is proud to be the Medical Support Specialist at the SSM Work & Wellness-Grande Clinics. With over six years of experience as a receptionist and audiology assistant for a non-profit clinic, she looks forward to being a resource for Associates through their care at the Grande Clinics. Brianna studied American Sign Language (ASL) and deaf Culture at Bloomsburg University in PA and is naturally a very organized, social person. She loves exploring new cultures and exploring the natural beauty of Wisconsin with her partner and two dogs.

Katie earned a Master of Science degree as a family nurse practitioner from Marian University in Fond Du Lac and is a board-certified Family Nurse Practitioner. She has over 6 years of experience as a RN working with psychiatry, hospice, as well as the hospital setting. Katie is excited to provide care to the Grande associates and their families and she looks forward to helping associates and family members achieve and maintain their health goals.
KAYLA STIEVE, MEDICAL ASSISTANT

Kayla has always been very passionate about helping others and because of this she started exploring the medical field working as a resident assistant in an assisted living home. She then became a CNA and worked in the nursing home setting for 3 years. After some years of being a stay-at-home mom, Kayla decided to do more with her dreams and she completed the medical assistant program at Moraine Park Technical College and has worked as a certified Medical Assistant for 3 years with experience in Pediatrics and Family Practice.
She is excited to be a part of the SSM Health and Wellness team at Grande. Kayla is motivated and eager to expand her knowledge as a medical assistant. She looks forward to building professional relationships with the patients as well as helping assist in guiding our patients in their journey towards improving their health and wellness to continue the activities they enjoy with their families and friends.
Outside of work she enjoys spending time with her family, taking trips up north, reading, going for walks in the woods, and fishing.
Ashley Rehbaum, Nurse Practitioner

Ashley graduated from the University of Wisconsin – Milwaukee with her Doctorate in Nursing Practice, specializing in family medicine. She is a board-certified family nurse practitioner. Ashley’s journey in nursing began with an Associate Degree in Nursing from Waukesha County Technical College, followed by her Bachelor of Science in Nursing from the University of Wisconsin – Milwaukee. She has over 10 years of experience as a RN working in an inpatient critical care setting. Ashley is excited to join the team at Grande Health and Wellness. Ashley is dedicated to building meaningful relationships with her patients and working collaboratively to achieve optimal health outcomes. Outside of her professional life, Ashley enjoys spending time with her family and friends, exploring the outdoors, and traveling.
