MEDICAL & PRESCRIPTION DRUG COVERAGE
Grande offers Associates the choice between two medical plan options. Associates can choose from a Preferred Provider Organization (PPO) plan or a High Deductible Health Plan (HDHP). Both plans are powered by UMR as the medical plan administrator. In addition, prescription coverage is included with both plans with CVS Caremark as the pharmacy benefits manager.
DENTAL COVERAGE
Healthy teeth and gums are important to overall Associate wellbeing. Oral health can be linked to other health conditions such as diabetes, and pregnancy or birth complications. To support dental health, Grande offers a comprehensive dental plan, powered by Delta Dental. Through Delta Dental, Associates have access to a PPO and Premier network and the plan allows for four cleanings throughout the year. While Associates can utilize any dental provider in the Delta Dental network, choosing a dentist in the network helps create additional savings.
VISION COVERAGE
Grande’s vision plans are powered by National Vision Administrators, LLC. Associates are offered two vision plans, a 12-month plan and a 24-month plan. The 12-month plan offers Associates the opportunity to enroll in the plan one year at a time. If enrolled in the 24-month plan, you must enroll in the plan for at least two years. Associates will not have the option to change their selection after year one. In addition, if you enroll in the 24-month plan, the coverage allows you to receive lenses, frames and contact lenses once every two years; whereas, if enrolled in the 12-month plan, you can receive the benefit each year.
