DIRECTIONS

The Grande Health and Wellness Clinic at the Rolling Meadows Facility is located just off of Trowbridge Road. The clinic is attached to the right side of the Rolling Meadows Facility. There is patient parking designated within the parking lot. The Clinic entrance is located on the far right of the Rolling Meadows Facility. The Health and Wellness Center is attached to the facility. Upon entering the waiting room, press the call button, and a provider will come to greet you.
